Key Responsibilities:
The CRT Specialist will be responsible for the following daily activities:
• Partnering with children and families to maximize community resources, services and natural supports
• Engaging and empowering families via the CRT process
• Collaborating with Interagency Teams for identified clients
• Developing Service Plans and Discharge Plans with the child and family and all Interagency Team members
• Communicating and working alongside county partners in conjunction with the CRT Supervisor as well as the Director of Community Outreach and Executive Director
• Facilitating Interagency Service Planning Team Meetings
• Providing direct linkages to services and resources to families
• Documenting all communication, case notes and dialogues as needed
• Completing client reports, packets and paperwork as needed
• Assisting CRT Supervisor in data collection and outcome measurement
• Supporting CRT Supervisor in fostering a supportive team environment and approach to services

| Overview:
The Children’s Review Team Specialist is a position that will work alongside Master’s level and Bachelor’s level mental health professionals. The Children’s Review Team (CRT) will be responsible for analyzing any referrals to Residential Treatment and Therapeutic Foster Care within the public mental health system in Chester County. This team will be responsible for facilitating the discussion regarding future services and resources for families. The CRT will act with the child and family to determine if all community resources have been exhausted. The CRT will make recommendations to the county about service gaps and the needs in the community.
