Job Title: High Fidelity Family Teams Facilitator
Position: Full Time-Regular

Overview:

The high fidelity family team’s process is a way to improve the lives of children with complex needs and their families by developing individualized plans of care. The individualized plan is child-centered and family-focused with maximum family involvement and variation depending on the needs of the child and family. The process focuses on strengthening the natural family, extended family and social supports for the child by involving them in the planning and implementation process. The Bucks County High Fidelity Family Teams Facilitator will work with youth and families being served through the project, providers, and county staff in Bucks County.

Basic Qualifications:
  • Master's or Bachelor’s Degree in the Human Service Field
  • Two years of experience providing services for children and families
  • Completion of the practitioners facilitators credential upon employment
  • Demonstrated excellent verbal and written communication skills
  • Proficiency with computer applications including Microsoft Word, Excel and Power Point
  • Bilingual in Spanish preferred

Responsibilities will include:
  • Ensuring that the phases and related activities of the High Fidelity Family Teams process are completed.
  • Collaborating with the youth and their family to engage them in the High Fidelity family Teams process and help them to identify and express their needs.
  • Collaborating with the team providing services and support to the youth and family and managing the team process.
  • Ensuring that the team mission and goals, and plan address the youth and family’s needs.
  • Monitoring implementation of the plan and communicating frequently with the youth and family to identify any emerging needs that may need to be added to the plan.
  • Teaching and transferring activities to the youth, family and natural supports by building and monitoring the youth and family’s self-efficacy.
  • Assisting the youth and family in identifying and strengthening their natural and social support system.
  • Monitoring satisfaction of all team members while strengthening and maintaining engagement.
  • Ensuring that the High Fidelity Family Team principles are upheld and the theory of change components are met.
 
Competencies:
  • Knowledge of all child serving systems.
  • Knowledge of families’ rights and responsibilities within all child serving systems.
  • Knowledge and understanding of the ten guiding principles of wraparound and their implementation in service planning.
