Job Title: High Fidelity Family Teams-Family Support Partner
Position: Full Time-Regular
Location: Hatboro, PA
Overview:
The high fidelity family teams process is a way to improve the lives of children with complex needs and their families by developing individualized plans of care. The individualized plan is child-centered and family-focused with maximum family involvement and variation depending on the needs of the child and family. The process focuses on strengthening the natural family, extended family and social supports for the child by involving them in the planning and implementation process. The Bucks County High Fidelity Wraparound Parent Support Partner will work with youth and families being served through the project, providers, and county staff in Bucks County.
Basic Qualifications:
- Be the parent of a child with complex emotional and behavioral needs
- Have successfully navigated child serving systems
- Life experience which allows you to possess a diverse view of what families need in order to have a better life
- High school diploma or equivalent required
- Knowledge and understanding of the ten guiding principles of wraparound and their implementation in service planning
- Ability to demonstrate problem solving skills within a collaborative approach
- Demonstrated strong verbal and written communication skills
- Proficiency with computer applications including Microsoft Word and email applications
Responsibilities will include:
- Partner with and support supervisor and facilitator to ensure that the phases and related activities of the High Fidelity Wraparound process are completed.
- Provide direct support and services to families in order to access natural supports and advocate for themselves
- Collaborate with the youth and their family to engage them in the High Fidelity Wraparound process and help them to identify and express their needs.
- Ensure that the team mission, goals, and plan address the youth and family’s needs.
- Communicate frequently with the youth, family and wraparound team to identify any emerging needs that may need to be added to the plan.
- Teach and transfer activities to the youth, family and natural supports by building and monitoring the youth and family’s self-efficacy.
- Assist the youth and family in identifying and strengthening their natural and social support system
- Attend scheduled High Fidelity Wraparound trainings and obtain required credentials
- Ensure that the High Fidelity Wraparound principles are upheld
Preferred Qualifications:
- Time management and organizational skills
- Ability to network and collaborate with service providers and professionals
- Knowledge of resources in Bucks and surrounding counties and the ability to access them
- Knowledge of and experience with child serving systems and family organizations
- Knowledge of families’ rights and responsibilities within the child serving systems.
- Knowledge and understanding of the ten guiding principles of wraparound and their implementation in service planning.
- Personality that is open to and supports engaging people from different cultures and backgrounds
