RTF Referral Management (RRM)
What is RRM?
The RTF Referral Management (RRM) program is an office-based service contracted by the Delaware County Office of Mental Health to coordinate referrals for Residential Treatment Facility (RTF) and Community Residential Rehabilitation–Host Home (CRR-HH) services for eligible youth. The program works closely with Community Care Behavioral Health (CCBH), treatment providers, families, schools, and other child-serving systems to ensure youth approved for these levels of care are connected to appropriate treatment as efficiently as possible.
RRM Specialists guide the referral process from approval through admission by coordinating provider searches, participating in Interagency Service Planning Team (ISPT) meetings, tracking referral progress, addressing barriers to placement, and collaborating with all members of the youth’s treatment team. The program also provides follow-up for up to 60 days after placement to help support a successful transition into treatment.
Who is eligible for RRM?
The RRM program serves youth and adolescents who reside in Delaware County and have been approved by Community Care Behavioral Health (CCBH) for Residential Treatment Facility (RTF) or Community Residential Rehabilitation–Host Home (CRR-HH) services. Eligible youth are generally up to age 18, or up to age 21 if they remain enrolled in school. Services are available to children and families of all races, ethnicities, cultures, sexual orientations, and gender identities.
Who can make a referral?
Referrals are typically initiated by licensed behavioral health providers through the Interagency Service Planning Team (ISPT) process. Once Community Care Behavioral Health (CCBH) approves the recommended level of care, the referral is assigned to the RRM program for coordination. Throughout the process, RRM collaborates with providers, families, schools, and other child-serving systems to support successful placement.
How do clients obtain services?
Youth are connected to RRM after receiving approval from Community Care Behavioral Health (CCBH) for Residential Treatment Facility (RTF) or Community Residential Rehabilitation–Host Home (CRR-HH) services. Following approval, the RRM team coordinates the referral process, facilitates provider searches, obtains required consent and release forms, and works with families and treatment teams to support admission.
